The SONGS Unit 3 Cycle 16 Refueling Outage and Steam Generator Replacement are scheduled to begin on October 10, 2010. In response to changes implemented for the issuance of security badges at the San Onofre Nuclear Generating Station, all Personal History Questionnaires (PHQ) must be received at the Bechtel New Hire Office no later than July 16, 2010.

There are two types of PHQ's used to conduct a background investigation based on the date of last unescorted access at a nuclear facility. Due to PHQ's being submitted in advance, a "cut-off date" has been established to determine which PHQ should be submitted.

· Long Form PHQ - The SCE Form AD (123) 511-1 (Long Form) Rev. 22, Southern California Edison San Onofre Nuclear Generating Station Personal History Questionnaire (PHQ) and Self-Disclosure is to be completed for an individual who has either never received 'unescorted access' or whose 'unescorted access' has been terminated greater than on year (365 days.)

·  Short Form PHQ - The SCE Form AD (123) 511-1A (Short Form), Southern California Edison San Onofre Nuclear Generating Station Personal History Questionnaire (PHQ) and Self-Disclosure is to be completed for an individual whose 'unescorted access' has been terminated 31to 365 days.)
